Athletics Physical and Participation Documents

Fill out and print the Preparticipation Physical History and Evaluation Form to take with you to the doctor.

Follow instructions to create and maintain a Final Forms Account for each athlete.

**Every athlete must complete the following steps prior to participation in their sport. (this includes during the summer)

Take a picture, then upload the Medical Eligibility form to the Final Forms website, and sign all documents on Final Forms website. (Both players and parents must sign on the Final Forms website)

Physicals are good for 1 school year and not 1 calendar year. Everyone must obtain a new physical each year after April 15th.

Current 6th and 7th graders may use this school year's physical for Spring Tryouts, but NEW physicals are required by June 1st for all athletes for any summer activities.

Physicals dated April 15th, 2025 through April 14th, 2026 expire on the last day of the 2025-2026 school year.

Physicals dated April 15th, 2026 or later will be valid until the end of the 2026-2027 school year.
