New Student Registration

In order to register at Station Camp Middle School, the following documents must be presented at the time of registration:

1. Birth Certificate (copy)

2. Two proofs of residency (acceptable forms below)

  • Driver's License
  • Legal document or Lease showing proof of residency
  • Vehicle Registration
  • Current Bill - Electric, gas, water, cable
  • Bank Statement
  • Employment Records
  • Voter Registration Card

3. Withdrawal paperwork from their previous school (Including IEP/Gifted information)

4. Current TN Immunization Record (7th grade needs to have TDAP and Varicella)

5. Parent/Guardian Driver's License

6. Custody Paperwork


The following forms will be filled out during the registration process:

1. SCMS Registration Form

2. Acknowledgement of Important Policies Updated 07/20/17

     Acknowledgement of Important Policies (in Spanish)

3. Student Residency Questionnaire

4. Home Language Survey

5. Tennessee Migrant Education Program- Occupational Survey

or Programa de Educación Migrante en Tennessee – Encuesta de Ocupación

6. Google Apps Permission Form 

7. Student Health Information (must be filled out at school on the original form)

8.  Military Service Information Form

9. Transfer Information
