Athletics Physical and Participation Documents

Parents please read the Parent/Guardian Letter

Fill out and print the Preparticipation Physical History and Evaluation Form to take with you to the doctor.

Follow instructions to create and maintain a Final Forms Account for each athlete. (Please select every sport that they will tryout for)

**Every athlete must complete these steps prior to tryouts/participation in their sport.

Physicals are good for 1 school year and not 1 calendar year. Everyone must obtain a new physical each year after April 15th.

Current 6th and 7th graders may use this year's physical for Spring Tryouts, but NEW physicals are required by June 1st for all returning players.

Upload Physcials and PPE forms to final forms and sign all documents (players and parents in final forms)
