Station Camp Middle School

EVERY STUDENT TRYING OUT FOR A SPORT MUST COMPLETE ALL OF THE FOLLOWING BEFORE TRYOUTS

Parents please read the Parent/Guardian Letter

Fill out and print the Preparticipation Physical History and Evaluation Form to take with you to the doctor.

Follow instructions to create and maintain a Final Forms Account for each athlete.

**Every athlete must complete the following steps prior to participation in their sport. (this includes during the summer)

Upload the Medical Eligibility forms to final forms and sign all documents in final forms. (Both players and parents must sign on the finalforms website)

Physicals are good for 1 school year and not 1 calendar year. Everyone must obtain a new physical each year after April 15th.
